Rooted Event Waiver

Event Name: Rooted in Wisconsin - A Cannabis Festival

Date & Time: August 29th, 2026. 11am - 9pm

Location: Washington Co. Fair Grounds, West Bend, 53095

Host: WiCannabisGroup LLC (WCG LLC)

Please fill out the following form.

1. Assumption of Risk; I, the undersigned, acknowledge and understand that attending the above-mentioned event may involve certain risks, including, but not limited to; physical injury, illness, death, or property damage. I voluntarily assume all risks, both known and unknown, associated with my participation in the Event, including exposure to infectious diseases or viruses (including COVID-19).

2. Release of Liability; I release W.C.G LLC and its associates from claims related to injury, damage, or loss incurred during the Event, whether caused by negligence or otherwise.

3. Indemnification; I agree to indemnify the Organizer against costs resulting from my participation.

4. Media Release; I allow the Organizer to use my likeness in recordings from the Event for promotional purposes without compensation.

5. Medical Treatment In an emergency; I authorize the Organizer to secure necessary medical treatment.

6. Governing Law; This agreement is governed by the laws of the State of Wisconsin.

7. Entry and Exit; No Carry-ins allowed. No open containers / beverages allowed to leave the festival. Be prepared to have personal belongings searched upon entry. Permitted items are determined at event discretion. I acknowledge weapons or harmful substances are not permitted in this festival.

I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I AM GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE.
